PRE REGISTRATION

Asheville Neurology Specialists utilizes electronic/paperless process in place of forms.

Prior to your appointment, you will receive an email to pre-register via a secure link.

Pre-registering prior to your appointment provides a convenient way for you to keep your information current. Having up to date clinical and demographic information allows us to serve you better.

MEDICAL RECORDS REQUESTS

For medical records requests, please fill out this Medical Records Release Form and fax the completed form to (828) 210-9319 or bring the completed form back to the office.

We charge a flat fee of $6.50 medical records processing fee for printed or mailed records. There is no processing fee for medical records transmitted electronically.
